造成忽略的原因可能為出血性或缺血性的腦中風、腫瘤或外傷,甚至退化性的腦病變也有可能造成忽略症候群。

急性腦部受損患者的研究發現,右大腦半球的受損(43%)較左大腦半球受損(20%)較常發生忽略的現象。

而若以受傷區域對應其臨床症狀,前葉(frontallobe)受損患者很難將其注意力從傷側移開(通常是右側);頂葉(parietallobe)與基底核(basalganglia)受損者其視覺定向及掃視的控制可能會有障礙;視丘(thalamus)主要匯整嗅覺以外之感官的輸出與主掌選擇性注意,除此之外,視丘與清醒度的調節也有很大關係,因此視丘受損易造成對側忽略,患者會很難注意到傷側對側物體。

其他相關研究也顯示,相較於完全性失語症患者和右側偏盲患者,忽略症患者的獨立生活能力較差,故忽略對於患者之影響不容小覷。

   忽略的症狀包括知覺無法注意(sensoryinattention)、動作無法注意(action-inattention)與空間忽略(spatialneglect)。

臨床上視覺之無法注意(inattention)與偏盲(hemianopia)常會造成混淆,區辨方式如下:視覺上的無法注意是轉動眼球就可看到,但偏盲是無論眼球如何轉動都無法看到傷側對側物體。

若一名患者傷及右腦,他可能看不到左側物體,但施測者若請患者將眼珠轉向右側即可看到左側物體,則此患者可能為視覺忽略而非單側偏盲。

知覺無法注意(sensoryinattention)可能出現無意識現象(unawareness)與消失現象(extinction)。

無意識(unawareness)是一種同側注意力的偏差,若一名患者傷及右腦,他將無法將注意力從右側移開。

而有消失(extinction)現象的患者(假設也傷及右腦)看到一堆樣式各異的物體時,他只會注意到在右側的物體,但如果給予的物體是有類聚(grouping)效果的,例如相同形狀,顏色等,讓患者認為這是『一個物體』,患者才會較注意到靠近左側的物體,可以利用此原則來消弱忽略患者的消失現象。

動作無法注意(action-inattention)也稱作動作忽略(motorneglect),臨床症狀類似於偏癱,但偏癱患者會嘗試動他的無力側,動作忽略患者則無移動其傷側對側肢體的跡象。

動作忽略分為以下形式:(以下以右大腦半球受損的忽略患者說明之)1. 半側空間動作失能(hemispatialakinesia):患者無法移動在傷側對側的肢體。

例如若把一名右腦受損患者肢體放在左邊會沒有動作,但把肢體移到右邊就可動作。

2. 方向性動作失能(directionalakinesia):動作失能的現象與肢體所在方位有極大關係。

有些患者無法將頭、眼甚至上肢移動至左側,但有些患者的頭、眼或上肢左右兩側皆可移動,但休息時會傾向於右側。

視覺的方向性動作失能常發生於大面積中風患者,此時就可能被誤解為gazepalsy。

3. 動作減少(hypokinesia):患者會有起始動作延遲的現象。

有可能同時發生於眼睛與肢體,也可能會受到方向的影響,右腦受損患者將物體移至左側的延遲現象比移向右側來的嚴重。

4. 動作不能持續(motorimpersistence):無法維持動作或姿勢。

測試方式為請患者閉眼20秒或是維持張口20秒,若其中一項可執行則請患者同時閉眼與張口維持20秒。

也可請患者維持上肢伸直20秒,傷側對側肢體幾乎很難維持伸直姿勢。

5. 辨距低估(hypometria):動作的幅度減少。

一名右腦受損患者剛開始可能無法轉向左側或是移動左側肢體,但隨著狀況的改善,患者可能慢慢的可移動左側肢體但起始動作緩慢,或是可動左側肢體但辨距低估只有小幅度的動作。
