Takotsubo cardiomyopathy (TTCM) is a form of stress ... with 37% occurring during anesthesia or surgery and 58% occurring postoperatively.AdvertisementSearchSpringerLinkSearchTakotsubocardiomyopathyanditsrelevancetoanesthesiology:anarrativereviewDownloadPDFDownloadPDFAbstractPurposeTakotsubocardiomyopathy(TTCM)isaformofstresscardiomyopathythatmayoccurintheperioperativeperiodandamongthecriticallyill.Therefore,anesthesiologistsshouldbeawareofitsdiagnosisandtreatment.TheaimofthisnarrativereviewisdescribethefeaturesofTTCManditsrelevancetothepracticeofanesthesiology.PrincipalfindingsTakotsubocardiomyopathyoccursinabout2-9/100,000personsinthegeneralpopulationannuallyandmayoccurinuptoonein6,700casesintheperioperativeperiod.Takotsubocardiomyopathyoftenpresentslikeanacutecoronarysyndromeandislikelycausedbyexcessivecatecholaminestimulation.Althoughitsearlycoursemaybecomplicated,morethan90%ofpatientssurvivetheacuteepisode.Areviewoftheliteraturerevealed131casesencounteredinmanydifferenttypesofsurgicalprocedures,with37%occurringduringanesthesiaorsurgeryand58%occurringpostoperatively.Comparedwithnon-perioperativecases,thispopulationinvolvedmoremales,wasyounger,lesslikelytohaveanobviousprecipitatingfactor,lesslikelytopresentwithchestpain,andlesslikelytoexclusivelyexhibittheapicalballooningpattern.Inaddition,perioperativeTTCMhadalowerejectionfractionandwaspronetohighermortality.Detectionisfacilitatedbyearlyechocardiography.AnesthesiologistsmayencounterTTCMinothersituationsincludingpatientsundergoingothernon-surgicalprocedures(e.g.,electroconvulsivetherapy),thosewithacutecentralnervoussystemconditions,thosewithpheochromocytoma,inothercriticalillnesses,andduringallergicreactions.ConclusionPerioperativeTTCMismorecommonthanappreciatedandshouldbeconsideredinanyhospitalizedpatientpresentingwithacutecoronarysyndromeand/orhemodynamicinstability,acuterespiratorydistress,aswellascardiacarrhythmiasandarrest.RésuméObjectifLacardiomyopathiedeTako-Tsubo(CMTT)estuneformedecardiomyopathiedestressq
